Editable Form 1095-C Now Available

If you’re an Applicable Large Employer (ALE) under ACA, your pre-populated, editable Form 1095-C PDFs are now available on My Resource.

How to access your form:

1. Log into My Resource
2. Go to the Reporting tab
3. Click HR Reports
4. Click ACA Dashboard Reports, View
5. Select 1095-C Pre-filled Part I Employee Information (.pdf)
6. Verify that your contact information displayed is correct
7. Click Submit
8. You’ll receive an email once your PDFs are ready to be viewed. You can also refresh the page to access the PDFs
